PCN Services
HASP PCN provides the practice with various different practitioners and services that patients can take advantage of, often without having to see a GP.
Social Prescribers
We have a team of social prescribers across the PCN . They have a wealth of experience in there field, and she can help people with social issues, carer support, debt management, finance, benefits, bereavement, isolation and many more issues. To talk to the social prescriber , just ask our reception team to arrange a referral.
Health and Wellbeing Coaches
We have a team of Health and Wellbeing coaches who cover the PCN . These are very experienced and well equipped to see people who wish to live a healthier lifestyle. They can help with diet, exercise and support you with managing long-term conditions. You can arrange a referral with her through the reception team.
Mental Health Support
We have a MIND, dedicated mental health support worker, who can help our patients deal with low level depression, grief and bereavement, stress and anxiety. A telephone appointment with her can be booked by the reception team.
First Contact Physiotherapy
We have two highly trained first contact physiotherapist who can assess and support people with new muscular skeletal concerns. You do not need to see the GP first, you can book directly with reception without the need for referral. We can also arrange joint injections for patients that may benefit from further intervention.
Home Visits
We have two community paramedics and a nursing associate at HASP that look after our housebound and care home patients. They visit patients at home on behalf of the practice and carry out weekly checks on our care home residents as well as holding minor ailment clinics across the surgeries.
